To take the survey, visit the link below on a computer or smartphone or scan the QR Code below before Friday, February 17, 2023 and enter your student’s UID as the access code. The survey is strictly confidential and administered by a survey company. The UID allows the district to organize the data by school, etc. Once you have logged in, you will be asked to begin the survey. If you do not have access to a computer or smartphone, please visit your school’s parent center or front office to take the survey.
